How do I make a claim?
If your provider has access to an automatic claims system (i.e. HICAPS or IBA HealthPoint), your extra’s claims can be paid immediately.
Or, you can download a claim form send it to Australian Unity Claims Department, Reply Paid 9945, Melbourne Vic 8060 or fax it to 1800 852 030.
Claims can also be submitted online via our Membership Online Services.
